About – Insurance Info and Providers

Lead Pediatric Physical Therapy is an out of network provider. This allows me to work with your child in a way that works best for you and your child, and helps your child progress optimally. This type of arrangement allows me to provide more personalized attention to you and your child, and to provide one on one sessions focused on what works and is meaningful.
Superbills can be provided, if you would like to submit them to your insurance to take advantage of your out of network benefits. We accept both HSA/ FSA cards, credit/ debit cards and Venmo.

The fee for the initial evaluation is $185 and 1 hour treatment sessions (after the evaluation) are $155. Half hour sessions are $95. Packages are available after the initial evaluation for a reduced rate, depending upon the number of visits purchased.

We are a TriWest Health Care Alliance in network provider. We are honored to serve military families in Fort Worth. The transition to TriWest has been rough to say the least, and LEAD Pediatric PT is not showing up online in the TriWest Provider Directory. Don’t worry, you can still have your child referred here. TriWest is able to enter referrals manually, but you must ask your provider to call the provider referral number at 844-866-9378. If you have a referral to another practice, and want to switch to ours, please call that same number. Please reach out with questions. I am here to help!
